When prescribing a medication or medications for a patient, there are several ways for us to accept them.  We can accept prescriptions via phone, fax, or     e-scribe. Below are some referral forms to help make sending prescriptions to us an easy process.  Simply download the form, type or print the appropriate information and fax to us at 414-906-0187.  Please note that faxed referral forms can only be accepted directly from the prescribing practitioner.
